Newest version at the top; keep an `## [Unreleased]` section above it.\n2. Sections, in this order, only if non-empty: **Added**, **Changed**, **Deprecated**, **Removed**, **Fixed**, **Security**.\n3. Write for users, not developers: describe impact, not implementation.\n4. Each entry ends with a PR/issue reference, e.g. `(#123)`.\n5. Version headings: `## [1.4.0] - 2026-01-31` (ISO date).\n6. Breaking changes go first under **Changed** or **Removed**, prefixed with **BREAKING:** and a migration hint.\n7.
