Gather: alerts, dashboards, deploy history (`git log`, CI runs), chat transcript.\n2. Build a UTC timeline: detection → escalation → mitigation → resolution.\n3. Quantify impact: duration, % of users/requests affected, data loss, SLO budget burned.\n4. Find causes with \"5 whys\" — expect several contributing factors, not one root cause.\n5. Write action items in three buckets: **prevent**, **detect faster**, **mitigate faster**.\n6.
